Payroll – W-2 Employee Information (US Only) 10-60-01

Due to the government requirement that Companies reporting W-2 information for 250 employees or more must report the employer-paid portion of the employees’ health insurance premiums, field 12 has been expanded to allow 4 miscellaneous fields. The amounts for reporting the employer-paid portion of the Employee’s health insurance premiums MUST be entered manually for each employee in one of the 4 miscellaneous fields “12a”, “12b”, “12c” or “12d”. The employer portion of the insurance amount needs to be entered with a code of “DD”. This amount is NOT taxable income to the employee.
